Duties:

• Support clinical team in advising clients on financial scheme applications, ensure timely update of renewal of means testing and financial assistance
• Support the clinical team in administrative processes in the delivery of services to clients and caregivers
• Manage onboarding process for new clients, assist in initial assessment appointment scheduling/ rescheduling, subsidies application, and address client inquiries for rehabilitation
• Assist in initial assessment of client/caregiver needs with basic case management under the direction of a case manager/ social worker
• Ensure client and caregiver’s ability to cope by helping to streamline and coordinate the range of community services available
• Assist client/caregiver with appropriate information and referral assistance
• Conduct joint home visits with a case manager/ social worker or the multidisciplinary team where necessary
• Support events or projects in relation to client and caregiver support as planned
• Participate in case discussion with the clinical team

• Consolidate, prepare, track relevant monthly reports and statistics
• Ensure that proper documentation on the systems and paper filing is completed on a timely basis
• Any other duties as assigned by the reporting officer

Working hours: Mon-Fri office hours
Location: West / Central

Requirements:

• Minimum Diploma in any healthcare related disciplines
