Job Description:

• Conduct thorough occupational/physiotherapy assessments during client admissions, periodic care reviews, and at discharge.
• Select and implement tailored outcome measures to gauge therapy progress and effectiveness.
• Collaborate with clients and stakeholders to develop personalized and achievable goals.
• Design evidence-based therapy interventions that prioritize individual client needs and preferences.
