Job Description:
2.1. Person-centred Care
a. Perform health screening such as blood glucose monitoring, measuring of blood pressure, nutritional status and BMI, and make appropriate referrals as necessary.
b. Perform nursing care such as administering medication and wound dressing as required.
c. Perform initial and 6-monthly full assessments (set consisting of various assessment forms stipulated) for all clients.
d. Assist in the development of individual care plans for all clients.
e. Implement and review individual care plans for all clients.
f. Plan, conduct and evaluate clients' participation in programme activities and manage accordingly.
g. Ensure comfort and safety of all clients.
h. Manage and escalate appropriately in the event of unexpected and/or abnormal changes.
i. Facilitate case conferences/discussions with members of the care team, including the Occupational Therapist, on a monthly basis.
2.2. Client and Caregivers Empowerment
a. Communicate and build rapport with clients and their families.
b. Involve clients, families, caregivers and/or relevant others as active partners to identify the needs, preferences and expectations of the clients.
c. Provide effective training and education to clients, families and/or caregivers according to self-management needs.
2.3. Nursing Practice Management and Operational Excellence
a. Work alongside the manager and care team in the achievement of targets and management of outcomes set for DCS annually.
b. Assess the eligibility of potential client for admission to DDC.
c. Maintain proper documentation of clients' progress and accurate collation of data (record of referral/enquiry/attendance etc).
d. Prepare statistics and reports monthly.
e. Guide and supervise the support care team and junior staff.
f. Recommend initiatives and implement quality improvement, evidence-based practice.
g. Identify risks/hazards and take appropriate safety and infection control precautionary measures as per established policies and procedures to ensure safety at the workplace.
h. Maintain the work environment and manage any issues as necessary.
i. Support early identification of health threat risks to clients through information and/or data gathered.
j. Identify and report risks and barriers to the safety of clients in accordance with organisational protocols.
k. Execute responsibilities as per emergency protocols in the event of a public health threat or emergency.
l. Comply with guidelines and policies to ensure client confidentiality and personal data protection.

2.4. Provide Emergency Care
a. Participate in medical emergencies or crisis situations as appropriate.
b. Monitor for early signs and/or deterioration of clients' condition with appropriate physiological assessments and observations.

a. Handle public enquiries of DDC and care of dementia clients.
b. Manage and monitor medical supplies, equipment and consumables inventory.
c. Participate in the training programme of a wide variety of community partners (e.g. interns and volunteers).
Job Requirements: Min VALID SNB & BCLS w min 3 yrs of relevant inpatient ward experience, preferably in acute/geriatric ward settings
