Job Responsibilities

  • Gather social and financial background information of residents and/or their families prior to the application of financial assistance schemes.
  • Update and maintain accurate records in the referral and enquiries database and information system.
  • Ensure timely submission of financial assistance applications such as Medifund, MFEC (Medical Fee Exemption Card), IDAPE (Interim Disability Assistance Programme for the Elderly), PGDAS (Pioneer Generation Disability Assistance Scheme), and others.
  • Act as the main point of contact and liaison between residents, families, and healthcare institutions to coordinate referrals and process applications for potential residents.
  • Ensure proper documentation of case files, referrals, and other relevant documents for effective case management and audit purposes.
  • Perform data entry and regularly update information to maintain accuracy of databases.
  • Handle administrative tasks including enquiries, referrals, and admissions.
  • Facilitate residents’ transfers to other facilities or healthcare institutions, as well as coordinate discharge processes.
  • Undertake any other duties or projects as assigned by the supervisor.
  • May required to be stationed in the office to handle incoming phone calls, walk-in enquiries, and other front-desk administrative duties as and when required

Requirements

  • Minimum 1 year of relevant working experience in social service, community service, healthcare, or related sectors.
  • Prior exposure to financial assistance schemes (e.g. Medifund, MFEC, IDAPE, PGDAS) will be an advantage
  • Diploma or Degree in Social Work, Social Sciences, Community Services, Healthcare Administration, or related disciplines preferred.
  • Comfortable liaising with residents, families, healthcare institutions, and external agencies.
  • Strong administrative and documentation skills, with attention to detail for case files and audit purposes.
