📍 Jurong
💰 $2,400–$3,500/month
🕒 5 days/week, including weekends & PH

💼 Responsibilities
• Conduct pre-admission screening for patients and caregivers
• Work closely with nurses to coordinate and provide holistic care for patients at home
• Monitor patients’ general condition and progress
• Identify and report abnormalities, care gaps or concerns to the appropriate nursing team
• Coordinate with healthcare providers, patients and families to support continuity of care
