What You'll Be Doing

• Review prescriptions and medication charts for appropriateness, dosing, interactions and potential adverse reactions
• Clarify and resolve prescription issues with prescribers and recommend suitable therapy
• Ensure accurate and timely dispensing of medications to patients
• Counsel patients and caregivers on drug therapy, side effects, storage and administration
• Provide outpatient pharmacy services such as medication review and anticoagulation services

• Degree in Pharmacy and above
• Registered with the Singapore Pharmacy Council

• Sound clinical knowledge and strong attention to detail

• Able to work shift duties for hospital roles, or flexible schedules for locum roles
