Working Hours

• Monday to Friday: 8.30am to 5.45pm
• Saturday: 8.30am to 12.45pm (Once every 4 weeks)

About the Role

We are seeking a compassionate and motivated Physiotherapist to join our Home Therapy team. You will be responsible for delivering patient-centred physiotherapy services within patients’ homes, supporting their rehabilitation goals and improving their overall mobility, function, and quality of life.

Key Responsibilities

• Conduct initial physiotherapy assessments using appropriate clinical outcome measures
• Develop and implement individualised care plans and rehabilitation goals for patients
• Provide physiotherapy treatment and interventions within home-based settings
• Educate and train caregivers on patient handling techniques, exercises, and home care management
• Recommend and prescribe suitable rehabilitation equipment where necessary
• Perform discharge planning and coordinate referrals to relevant healthcare or community support services
• Participate in monthly home therapy and multidisciplinary team meetings
• Conduct therapy in-service sessions and attend training programmes organised by the department
• Maintain accurate clinical documentation and support administrative duties as required

Requirements

• Degree or Diploma in Physiotherapy recognised by the Allied Health Professions Council (AHPC)
• Full registration with the Allied Health Professions Council (AHPC)
• Strong interpersonal and communication skills
• Able to work independently in conducting home-based assessments and interventions
• Experience in developing patient-centred and customised care plans
• Passionate about rehabilitation and community-based patient care
