Location: Bukit Merah View | Islandwide Case Management (Transport expenses reimbursed)
Working Hours: Mon–Fri, Office Hours
Salary:

Social Worker: $3,800–$4,700

Senior Social Worker: $6,000–$6,500

Job Scope:

Casework & Counselling

• Conduct assessments, care planning and interventions for seniors and caregivers.

• Develop, implement and review individualised care plans.

• Build rapport and provide psychosocial support to clients and caregivers.

• Advocate for clients' needs and coordinate appropriate community resources.

• Manage case referrals and maintain accurate case documentation.

• Participate in case conferences and multidisciplinary team (MDT) meetings.

• Monitor and evaluate coordinated services and case outcomes.

Care Coordination & Community Support

• Coordinate care services to support seniors to remain in the community.

• Provide information on relevant assistance schemes and care options.

• Support seniors and caregivers in advance care planning and end-of-life discussions.

• Collaborate with internal teams, healthcare professionals and external agencies.

• Conduct community outreach activities and support other ad-hoc initiatives where required.

Requirements:

• Graduate Diploma or Bachelor's Degree in Social Work.

• Registered Social Worker with the Singapore Association of Social Workers (SASW).

• Experience in case management, counselling, care coordination or community-based services will be advantageous.

• Comfortable travelling to different locations across Singapore for client-related work.
