Key Responsibilities

• Facilitate Advance Care Planning (ACP) discussions with patients, families, and nominated healthcare decision-makers to understand and document patients’ care preferences.
• Ensure timely accessibility and proper maintenance of records for healthcare professionals and care providers.
• Provide follow-up support to patients and families for review and updates of decisions when required.
• Advocate and promote awareness of among patients, caregivers, healthcare professionals, and community partners.
• Assist in organising events, training sessions, meetings, and community outreach activities.
• Collaborate with multidisciplinary teams including doctors, nurses, allied health professionals, and community partners.
• Support quality improvement projects, research initiatives, and process enhancement related to services.
• Contribute to educational materials, newsletters, and outreach initiatives to increase awareness in the community.

Requirements

• Bachelor’s Degree in Psychology, Nursing, Social Work, Counselling, Allied Health, or related disciplines.
