Location: West
Duration: 1 year Contract
Salary: $12/hr

Working days and hours:
- 5 days a week
- Mon-Fri, 8am -430pm
- Sometimes will be activated 1 day on weekends if they have event

Job Description:
- Manage the one-stop Healthier SG booth and provide assistance to patients for Healthier SG.
- Assist with HSG assisted enrolment through small group sharing and one-to-one engagement with patients/residents in outreach settings such as hospitals, designated booths or community settings, via the mobile app or system application.
- Explain to patients the benefits of HSG initiatives and the importance to enrol, make the first visit to review Health Plan with the Primary Care Provider, fulfil the preventive health recommendations and follow up for review annually.
- Attend to HSG related enquiries from patients and caregivers.

Job Requirements:
- Min N/O/A Level
- Comfortable being customer service oriented
- Proficient in Microsoft Applications
- Comfortable standing for 3-4 hours minimally throughout the day
- No exp. needed, training will be provided
